Benefits
The Hacienda La Puente Unified School District offers excellent medical, dental and vision insurance to eligible certificated employees and their dependents. The district pays up to $9,500 for medical, dental and vision insurance for employees and eligible dependents. Please check with our Benefits Office (626) 933-3861 for your exact coverage. The plans available are:
MEDICAL
Blue Cross Prudent Buyer Incentive Plan
Blue Cross fee-for-service: basic and major medical benefits; $500 deductible per person ($1,000 per family); 70% of usual, customary and reasonable charges. Freedom to choose hospital and physician.
Prudent Buyer Provider Plan: basic and major medical benefits; $25 co-payment for doctor office visits; $10, $20, $50 OTC prescription plan; Mail order prescription (3 months) is $20/40/90; $500/$1,000 deductible for major medical benefits; 90% of negotiated fee if using Prudent Provider.
Blue Cross Power Health Fund - New Plan effective January 2007 - Please contact Benefits Office at ex 3861.
Kaiser
Restricted to Kaiser medical and hospital facilities. Kaiser has physicians in all major medical and surgical specialties as well as family practice physicians. Most charges are paid at 100%; $10 co-payment for doctor office visits. There is a $50 co-payment for emergency room treatment unless admitted to the hospital. Prescription co-payment is $5.00 or $15.00 per prescription for covered pharmacuticals.
Pacific Care
Restricted to contracted medical groups, clinics and physicians in Southern California. Most charges are paid at 100%; $10.00 co-payment for doctor office visits; $5, $15, $25 OTC prescription plan. Mail order prescription (3 months) is $10/30/50. There is a $50 co-payment for emergency room treatment unless admitted to the hospital.
DENTAL
Delta
Choice of licensed dentist. Pays 80% of dentist's filed fee for most services; 50% of filed fee for dentures & bridge work or of usual, customary & reasonable charge if non-participating dentist is used. Annual maximum benefit of $1,500. Dependent benefits are available with employee paid monthly premium. Orthodontia available with co-payment for dependent children.
Delta Care/PMI
Restricted to contracted dentists. Orthodontia available with co-payment of $1,600 for children to age 19 and $1,800 for adults (excludes start up fees). This is employee-only coverage with the option of employee-paid family coverage.
VISION
Vision Service Plan
Plan provides for a vision examination every 12 months and, if needed, lenses and frames every 24 months. There is no charge for these services if you use a participating Vision Service Plan provider.
